New Patients


Registration for new/temporary patient AFTER 11AM- 6PM  MONDAY – FRIDAY. If this time is not conveinent please speak with a member of our reception team.

New patients wishing to join our practice will need to complete a form for NHS Contractor Services( GMSI Form), this will enable us to gain access and transfer your medical records. You will also need to fill in a New Patient Registration Form to help us provide you care before your medical records become available to us. These forms are available at reception and online( below)

Please be advised you will need a form of ID and proof of address when registering. If you haven't got either, please speak with a member of our reception team.

Children under the age of 5 years old, the practice will need to see the child's Red Book or any Immunisation history upon registration.


New patients who are on regular medication (including contraceptive pill) or having treatment for chronic conditions must make an appointment with the nurse/doctor before their prescriptions can be reissued. It would be helpful if new patients would bring the repeat prescription printout to this appointment, together with any relevant letters from hospital specialists.

Online Services

You can register for online services to book APPOINTMENTS, order REPEAT PRESCRIPTIONS or access to your MEDICAL RECORDS. (Patients can view, export or print summary information from their medical records relating to medications, allergies, adverse reactions and additional data that the doctor has agreed the patient may have access to on-line.)
