Sick (fit) notes

Important instructions on sick notes – PLEASE READ FIRST.

If you’ve been off sick for more than 7 days

To request a sick note:

If you’ve been off sick for 7 days or less

You should self-certify for the first 7 days of any illness with your work place.

If you’ve been off sick for more than 7 days (and have completed your own self certificate) OR this is a ongoing/ continuation GP sick note

If this is a new/first certificate, then you must make sure you have a telephone/ face to face appointment with a doctor to discuss your sick note for the 8th day of your illness.  

We will NOT back date any periods where there is no clear recent clinical documented consultation about your illness.

Also, you can use the link below If this is an ongoing sick note i.e. continuation from a previous sick note given by the GP surgery.

PLEASE be aware that we may not always be able to give you long periods of sickness time if it is not clinically justified.

ALSO please put in your FIT note request on the day you require it to start from (our computer system will not allow us to forward date any FIT notes).

We will contact you e.g. SMS, email or telephone call if there are any problems and ask you to make an appointment if needed to discuss this. 

Your sick note will be sent via SMS or email.
